Fixed #178 New Ffmpeg encoding related bugs.
Added game event transmission (optional) to mirv_pgl (should be backwards-compatible) and updated the misc/mirv_pgl_test/server.js node example accordingly. (There's quite a lot of data you can not get with CS:GO game state integration at the moment, e.g. you can tell where defuse kits have been dropped and many other things.)
